Duties/Responsibilities: 1. Participate in research utilization activities and integrate findings into practice. 2. Develops an individualized plan of care based on the assessments. 3. Assesses all aspects of the patient's pain and develops an individualized pain management plan. 4. Anticipates, prevents and treats undesirable symptoms or secondary symptoms. 5. Supports, instructs and educates the patient, family and caregiver. 6. Documents problems, nursing assessments, appropriate goals, care provided, care interventions and patient and family outcomes. 7. Coordinates all patient and family services with the team. 8. Maintains the dignity of the dying patient. 9. Supports the patient's and family's unique spiritual and cultural beliefs. 10. Provides holistic, family-centered care across treatment setting to improve the quality of life.
